Short answer
Fine alpaca (under about 25 microns) feels soft against most people's skin. Coarser alpaca can prickle, as can any fiber over about 30 microns.
Itch is caused by the ends of coarse fibers pressing on the skin. Because alpaca has no lanolin, people with lanolin sensitivities often find it more comfortable than wool.
